Question Re: NSW "Mobility Parking Scheme" Permits

Does anyone here know, if the driver applying doesn't have Condition "S" (spectacles) or any other notifiable eye condition - do they still need an optometrist to sign off on their vision? The form is not really clear. I understand that a known doctor has to sign off on the other guff about fitness to drive, and that cannot be avoided. 

As I get to do the Ubering and waiting, if I can avoid having to take someone for yet another bloody paramedical appointment, I'm keen to do so.
